​I'm Sophie, the founder of Meliora Nurse Care Management. I know firsthand how overwhelming the healthcare system can feel. My own journey has taken me through countless specialists, tests, and unanswered questions, navigating complex diagnoses, coordinating between providers, and managing the everyday realities of living with chronic conditions.

Because I’ve been in the patient’s chair, I understand the frustration of repeating your story, the exhaustion of managing symptoms while juggling appointments, and the importance of having someone who sees the whole person, not just lab results or diagnoses.

That’s why I created Meliora Nurse Care Management: to be the advocate, guide, and partner I wished I’d had from the start. I bring not only professional expertise as a nurse care manager, but also deep compassion and understanding born from personal experience.

As a care manager, I have

• Helped clients get insurance coverage for needed out of network services

• Provided reminders of upcoming appointments and pending insurance payments, ensuring a greater percentage of attended appointments and continued insurance coverage

• Advocated successfully for clients to get reimbursed for covered services that they paid for out of pocket

• Helped clients notice patterns that were both detrimental and helpful to managing their chronic condition

• Attended appointment with clients, ensuring important questions are asked and everything is explained in a way the client understands
